Unless otherwise disclosed during collection and with parent or guardian consent, Jevitty does not knowingly collect Personal Information from children under the age of majority.
Jevitty collects Personal Information when you register for the Service or when you use the Service. Depending on how you use the Service, we collect different kinds of information about you. This information may include:
Any information that identifies you as a patient of a health care provider or any other information regarding your health may be PHI. We will only use or disclose PHI as permitted or required under Canadian law and the Personal Information Protection and Electronic Documents Act (S.C. 2000, c.5). PHI is collected in the Application via your mobile device, using Apple’s Health Kit functionality, and other means. This PHI is securely stored and processed for display in the Jevitty Application and used to provide you with feedback on your health, provide recommendations, information and product information aimed at informing your decisions about a healthier lifestyle.
PII is any information that can individually identify you, and includes your name, and contact information, such as e-mail address, telephone number, or postal address. This may be provided by you to us, at the time of sign up and user account creation.
Non-personally identifiable information includes information that does not personally identify you, but it may be linkable to you. If non-personally identifiable information is directly linked to PII, it will be considered PII while it is so linked to PII. Aggregated and de-identified information is not considered PII, because it is not possible to identify you from such aggregated and de-identified information.
We also may collect Personal Information about you that we may receive from other sources or from our offline interactions with you to, which among other things, enable us to verify or update the information contained in our records and to better customize the Service for you. We may combine Personal Information gathered from multiple parts of the Service, or Jevitty’s other systems and services, into a single record.

These entities may have access to aggregated or non-aggregated Personal Information, if needed to perform their functions. If such access is required, we will limit disclosure of Personal Information to only that Personal Information that those third parties require, and the third parties will be contractually obligated to maintain the confidentiality and security of that Personal Information. They are restricted from using, selling, or distributing this data in any way other than to provide the requested services to the Service or as required by law.
We may disclose de-identified versions of your Personal Information and other data (“De-Identified Information”) in aggregate forms with partners, investors and contractors for any purposes related to our business practices. Permitted uses of your De-Identified Information may include but are not limited to, product development, marketing and disclosures required by law, for example, to securities regulatory bodies and their reporting requirements under applicable laws. We may, at any time, sell, transfer, or assign any or all of our rights to our Canadian business, including our interests, rights or obligations regarding your accounts with us. If we do so, we may share your Personal Information with prospective purchasers, transferees or assignees. In such transactions, customer information shall be treated as a transferred business asset. In the event that Jevitty (or all, or substantially all, of its assets) is acquired by a third party, Jevitty’s customer information will be considered a transferred business asset.
We take steps to ensure that your personal information is as accurate, complete and up to date as is necessary for the purposes for which it is to be used.
